Intake Assessment

Visit the "How to Participate" page for exact steps/paperwork to become a client. During the intake process, you will meet with one of our office staff to welcome you to our programs and to share information to help you live well with Parkinson's. You will be assessed by a speech pathologist and a physical therapist. Please bring someone with you and allow 2 hours for this process.

Participation

Monthly Fee for all Programs - $50

Care Partners can participate in some of our classes at no extra cost.
